There are definite signs
and actions that geriatric care managers look for in terms
of possible moves, or bringing in additional help to the
current living situation of seniors. Although we can list
the signs, it must be remembered that each case has to
be assessed individually as there are degrees within each
sign. That said, here is a list of some potential areas
to review: |
- EATING…What
types of food, quantity, variety, freshness?
- BATHING…How
often, can they do it by themselves or with assistance?
- DRESSING…Are
the clothes clean, do they change clothes or wear
the same thing daily?
- TRANSFERING….Can
they get into their bed, out of a comfortable chair
safely?
- COOKING AND FOOD PREPARATION…What
is the condition of the kitchen?
- SHOPPING…Do
they make appropriate decisions?
- TAKING MEDICATIONS…Are
there pills scattered thru the house, is there a list
of medications to be taken and when?
- TELEPHONE USEAGE…Can
the phone be used properly?
- HOUSEKEEPING…Is
the house dirtier than usual, are floors cluttered?
- DRIVING…How
often are they getting lost, can they verbalize directions?
- PAYING BILLS AND MONEY
MANAGEMENT…Are they paid timely,
is the checkbook balanced, are papers filed appropriately?
- LAUNDRY…Are
clothed scattered around, is the washing machine being
used properly, is there soap?
- SAFETY…Can
they use the stairs, are their grab bars in the shower,
can the person exit the house on their own in the
event of an emergency?
- EMOTIONAL.
Is the person depressed or sad, are they in constant
grief about losses, do they have current interests
and activities, are they anxious or calm, what is
their view on life and their
world?
- SOCIALIZATION…Do
they get out, do they participate in activities that
they previously enjoyed, do they still attend their
place of worship?
- HEALTH…Last
but not least are all the health concerns being addressed?
